Consumer Notice & Client Rights
To maintain transparency and comply with Texas state regulations, this page outlines your rights as a consumer, how to request your health care records, and the appropriate avenues for submitting professional or business complaints.
1. Board Complaint Notification
Pursuant to 22 Texas Administrative Code § 884.31, notice is hereby given to the public that complaints regarding the services provided by a licensed mental health professional can be directed to the state licensing authority.

2. Business & Consumer Protection Complaints
If you have a commercial, financial, or deceptive business practice complaint against this practice that does not pertain to clinical misconduct or licensing violations, you have the right to notify state consumer advocates.

- Avenue to File: You may file an online report directly via the Texas Office of the Attorney General Consumer Protection Page.
3. How to Request Your Health Care Records
In accordance with Texas healthcare transparency rules, you have a legal right to access or obtain copies of your protected mental health records. To process your request safely and legally, please follow these steps:
- Step 1 (Submission): Send a written, signed request. You may send your request via standard mail.
- Step 2 (Verification): Your request must include your full legal name, date of birth, and specific parameters of the records you are requesting (e.g., progress notes, treatment plans, or billing records). You must also include the address where you want your records sent.
- Step 3 (Delivery): Records will be prepared and delivered securely within the legally required timeframe.
